HealthyCircles Connects to the Surescripts Network
to Reduce Hospital Readmissions

HealthyCircles will leverage Surescripts' Medication History Acute service to provide hospitals and their extended acute care facilities a solution to address medication reconciliation, post-discharge care coordination and reducing hospital readmissions.

SAN DIEGO - August 8, 2011 -

Today, HealthyCircles™(www.healthycircles.com), an integrated suite of online Professional and Consumer portals, announced it will connect to the Surescripts network to access Surescripts' Medication History Acute service. The combination of HealthyCircles' Care Coordination capabilities and Surescripts' medication history service will offer hospitals and extended acute-care facilities a powerful toolset to manage the discharge process, coordinate transitions of care and reduce 30-day readmission rates.

"Secure, electronic access to medication history can improve care and save money by reducing the time it takes to gather patient information and by enabling providers to make more informed clinical decisions," said Cris Ross, Executive Vice President and General Manager for Clinical Interoperability at Surescripts. "With the Surescripts service, a patient's medication history is available at the time of hospital admission, points of transition within the facility and upon discharge. This service provides a more seamless patient experience and facilitates the coordination of care."

Surescripts' Medication History Acute service allows prescribers and authorized staff in acute-care settings to query and receive aggregated details for up to a year's worth of patient medication history from payer and pharmacy records, representing over 240 million patients. The HealthyCircles Transition of Care service is designed to support the needs of acute-care facilities, hospital emergency departments and critical access hospitals, including those facilities that are rural or small.

"The Surescripts Medication History Acute service will significantly streamline and deliver cost efficiencies associated with the acute-care facilities' discharge processes and care coordination efforts," stated James Mault, MD, FACS, Chairman and Chief Executive Officer of HealthyCircles. "The Surescripts service available with HealthyCircles will enable hospitals a simple solution to manage transitions of care and to reduce the clinical and financial burden for all Care Team members involved in the patient's care."

The HealthyCircles Connected Health Platform securely connects patients, family caregivers and health care professionals via a proprietary record sharing and communication infrastructure. The ability to create and deliver interactive health programs, along with exception management tools for care management and care coordination, allows for the early detection of potential adverse events and changes in patients' clinical status. HealthyCircles incorporates familiar social networking technologies to ensure the patient, professional and caregiver engagement is a simple, collaborative experience.
