As I reflect on 2025 and the Surescripts Annual Impact Report, one truth stands out: the Surescripts Network Alliance accomplished what healthcare can only achieve when we work together.

This isn't just a statement of optimism. The data in the Annual Impact Report is proof. The numbers tell a powerful story about what happens when thousands of health systems, pharmacies, life science companies, payers and technology partners align around a common mission—putting patients first. With this report, you’re seeing evidence of that alignment.

The Surescripts Network Alliance grew stronger in 2025, not just in size, but in impact. Together, we didn't just process transactions. We transformed how health intelligence moves through the American healthcare system. We built the digital infrastructure that enables a patient's care team—whether they're across the hallway or across state lines—to make informed decisions in real time.

1. Health intelligence transactions in the billions

30.5 billion transactions were exchanged in 2025, which represents a 12% increase from 2024. This includes sending e-prescriptions, optimizing those prescriptions with benefit information, managing prior authorizations, accessing medication histories, exchanging clinical direct messages, and more. Collaborative and efficient intelligence was shared across nearly all electronic health records (EHR) vendors, pharmacy benefit managers (PBMs), pharmacies and health systems.

At the heart of it all are patients and the healthcare professionals who care for them.

2. Adoption of tools that make prescribing simpler: CancelRx, RxChange and RxTransfer

Did the treatment plan change? Did a coverage barrier get in the way? Or is the medication itself out of stock or unavailable from the pharmacy? These scenarios require quick action to keep care on track. Surescripts is making prescribing simpler for clinicians.

With CancelRx, prescribers can instantly alert the pharmacy when they discontinue a patient’s medication. RxChange allows pharmacists to recommend an update to a patient’s prescription and get approval from the prescriber. And when a pharmacy needs to forward a prescription to a different location, RxTransfer makes it easy.

CancelRx saw 277.8 million requests in 2025.

3. An upward trend in patient-specific benefit information: Real-Time Prescription Benefit (RTPB) trends

Real-Time Prescription Benefit helps prescribers understand not only what a patient’s benefit plan covers, but also the amount the patient will pay at their choice of pharmacy. In 2025, prescribers used Real-Time Prescription Benefit 1 billion times, saving their patients tens of billions of dollars collectively, and over 900,000 prescribers are using Real-Time Prescription Benefit to achieve those savings for patients.

4. Slashing prior authorization wait times and hassles: Touchless Prior Authorization performance

Getting patients started on an appropriate treatment that is covered by their health plan should be a quick and simple process. But one of healthcare’s biggest pain points can slow down the process: prior authorization. In 2025, Surescripts continued partnering closely with PBMs and health systems to implement the timesaving technology offered by Touchless Prior Authorization. Imagine the acceleration possible in a treatment plan when a prior authorization is approved in 18 seconds.

5. Interoperability makes healthcare work: Interconnect™, QHIN® designation & clinical data exchange growth

From patient outcomes to value-based care initiatives, there are many reasons to work closely across organizations to manage patient health—and all of them require interoperability. In 2025, Surescripts cofounded The Sequoia Project's Interoperability Matters Pharmacy Workgroup to expand pharmacy participation in clinical data exchange and help give pharmacists more power to fill care gaps with expanded clinical services.

In this chart, we focus on Clinical Direct Messaging which lets healthcare professionals securely exchange protected health information. In 2025, 1.08 million healthcare professionals and organizations (an 11.8% increase from 2024) used Clinical Direct Messaging to coordinate care.

6. Mapping medication trends

As the nation’s most powerful health intelligence network, Surescripts has unique insight into prescription trends and barriers across the U.S. One medication trend our Data & Analytics team tracked in 2025 is the continuing rise in prescriptions for GLP-1 agonists (GLP-1s).   

Data from 2025 shows continued growth in all but two U.S. states, as well as Puerto Rico.
