Proactively improving adherence & safety for patient populations

Mount Sinai Health System in New York adopted Medication History for Populations with patient notifications to help manage the care of more than 10,000 patients supported through its population health programs. Their care navigators are notified in near real time when their patients abandon or run out of refills, get a prescription from a new provider, or generate other prescription activity that could foretell nonadherence or an adverse drug event. With these insights, they can reach out to patients before any harm results.

“Getting these Surescripts notifications allows us to know the right time to contact these patients and help facilitate their education and engagement with their providers.”

Robert Dean Care Navigator, Mount Sinai Health System

“Having these notifications pushed to us that are reliable, gosh, it’s just amazing. Then we don't have to have navigation staff sitting with Excel and these different tool sets that are not convenient."

Ruchi Tiwari Pharm.D., Executive Director of Ambulatory Pharmacy & Population Health, Mount Sinai Health System
